Case Manager, Marshall Hospital/Sierra Foothills AIDS Foundation
Administered community AIDS program during transition period from hospital-based to private agency. Case Managed 35 clients in both home and hospital settings. Assisted clients/families with medical, financial, psychosocial, education and end-of-life aspects of disease. Consulted with Medical and Nursing staff in treatment planning.
Client/Family liaison in hospital and AIDS Foundation settings.
Community Health Advocate/Community Case Management
Provided Case Management for 40 senior clients in home and hospital settings. Initiated and created community volunteer network among hospital, county community services, community hospice agency and community church health outreach.
Human Resources
Employee relations, conflict resolution, workman’s comp rehabilitation services, recruitment/advertising, benefit administration, liaison between employees and insurance companies, staff training for 100 bed hospital. Initiated, created and implemented hospital-wide public relations program for directors, supervisors, employees and community members, including running focus groups and disbursing, receiving and evaluating hundreds of patient questionnaires.
Clinical Administration
Directed central communication flow on Medical/Surgical, Intensive Care, Emergency, and Obstetrics units. Coordinated high volume medical staff communication with patients, families, and nursing staff, at times in the face of life-threatening emergency. Implemented MD patient- specific medical and/or surgical orders. Simultaneously monitored cardiac function of up to 16 patients. Diagnosed irregular and life-threatening heart rhythms. Coordinated medical, nursing and ancillary staff communication in the event of cardiac arrest as well as well as all medical emergencies arising on unit.
EDUCATION
